Preliminary application
Download the preliminary application, complete it, and file it with the Scholarship Committee by May 1.
How to apply
- Download the Preliminary Application (PDF).
- Complete every section. We require a Social Security number for our records.
- File it with the Chairperson of the Scholarship Committee no later than May 1. Only one application may be filed per student.
The scholarship is open to high school seniors whose parent is an active, retired or fallen sworn law enforcement officer, within the jurisdiction of LEO Only.
